Who We Are
The Georgia Association for Primary Health Care, Inc. (GAPHC) was founded as a not-for-profit corporation in 1979 through the cooperative efforts of a broad coalition of primary health care advocates in our state.
Throughout its history, GAPHC has engaged in a strong public advocacy agenda in support of its mission: to improve access to comprehensive primary health care services for all medically underserved Georgians and support the continued development and expansion of community-based health center practice systems throughout the state. Our Purpose as a Primary Care Association (PCA)
The National Association of Community Health Centers (NACHC) works closely with state primary care associations to fulfill the shared mission of delivering quality health care to those in need.
In partnership, NACHC and primary care associations provide education, training and technical assistance to new and established health centers. NACHC and primary care associations are working to expand the resources and capacity of health centers to expand care from today’s 16 million patients to more than 30 million patients by 2015.[1] What is the Community Health Center (CHC) Program?
What is the The Community Health Center (CHC) Program? The Community Health Center (CHC) Program is a Federal grant program funded under Section 330 of the Public Health Service Act to provide for primary and preventive health care services in medically-underserved areas throughout the U.S. and its territories. CHCs provide family-oriented primary and preventive health care services for people living in rural and urban medically underserved communities. CHCs exist in areas where economic, geographic, or cultural barriers limit access to primary health care for a substantial portion of the population; and they tailor services to the needs of the community.[2]
